The Recovery Assistance Program offers community access and capacity building support to people with a mental illness. If people do not have a formal diagnosis, we support them to receive that diagnosis and create a recovery plan in order to set and achieve personal goals.

Eligibility Criteria:
People 18 years and over with a diagnosed mental illness
Assessment Procedure:
• Phone assessment – quick questionnaire to determine eligibility. During this call, the face-to-face appointment will be booked.
• Face-to-face assessment – questionnaire to support TeamHEALTH to gain an understanding of the support the participant requires and provide an overview of their current situation.
• Allocation of Key Worker if the participant would like to progress with the intake process and work collaboratively with a support worker to achieve their goals.
