Psychosocial Support Program

The Psychosocial Support Program supports people who are experiencing a mental illness to assist them achieve their goals by using a recovery focused and strengths-based approach. Our Mental Health Recovery Workers help people to focus on wellness and provide practical assistance in achieving personalised goals. Workers can assist participants with the access process and transition to the NDIS.

Eligibility Criteria: 
• 16 years + • Catchment area: Urban and rural regions; Darwin, Palmerston, Litchfield and Maningrida. • Has a diagnosed mental illness or experiencing mental health concerns. • Willing to engage with a Mental Health Recovery Worker to work towards their goals. • Not engaged with another support service who are providing the same type of support.
Assessment Procedure: 
• Phone assessment – quick questionnaire to determine eligibility. During this call, the face-to-face appointment will be booked. • Face-to-face assessment – questionnaire to support TeamHEALTH to gain an understanding of the support the participant requires and provide an overview of their current situation. • Allocation of Key Worker if the participant would like to progress with the intake process and work collaboratively with a support worker to achieve their goals.
