Eligibility Criteria:
• Must be over the age of 18 with mental health concerns.
• Must be engaged with a support service such as TeamHEALTH’s Recovery Assistance Program.
Assessment Procedure:
Complete a referral which will be assessed by TeamHEALTH's RAP team and CHS. Referral forms can be found on our website. Alternatively, contact TeamTALK, TeamHEALTH's Phone Support Line, and our friendly workers can assist you to complete a referral form over the phone.
