Carers Gateway Services provided to carers providing care to a family member or friend who has disability, mental illness, chronic condition, terminal illness or who are frail
Assessment Procedure:
CARER GATEWAY SERVICES
Registration process takes approx. 10 minutes and can be done over the telephone.
Following registration if the carer requests additional services they can make a further appointment to complete a Carer Star to identify care needs and this takes approx. 45 mins and can also been completed via telephone.
AGED CARE SERVICES
Carers NT provide CHSP services - Flexible Respite, Overnight Cottage Respite, and Community Based Day Respite Services
NDIS SERVICES
Carers NT a registered service providers under the NDIS.
