34 South Terrace Transitional Housing Program is envisaged to become a culturally respectful, transitional housing and support program to prepare participants for success in the management of longer-term accommodation options. Accommodation and in-reach support is provided for up to 18 months. Exiting pathways for each client will be considered on a case-by-case basis depending on the client's individual circumstances.

Eligibility Criteria: 
Participants will be those who are: Homeless; residing in crisis accommodation; or living in overcrowded accommodation. On the Public Housing wait-list. Willing to engage with the service and participate in case management and other support services, where applicable. Willing to transition to independent housing on completion of the program. With respect to the family groups, first priority will be given to young parents, as it is recognised that support to young parents provides positive outcomes for both the parents and their children.
Assessment Procedure: 
Intake will be part of the referral process to the program.
